First Visit

What to expect at your first visit:

Please complete our online registration with the link sent to you by our front desk team before you arrive for your first appointment. The online registration contains general patient information, medical history, and other documents for you to read and sign prior to your appointment. You will receive a separate email from us with your physical therapy benefit information. Our online registration links are below if for some reason you did not receive an email from us.

PLEASE BRING THE FOLLOWING TO YOUR APPOINTMENT:

  1. Any applicable co-payment (We accept Visa, Mastercard, Apple Pay, Google Pay, or Personal Checks).
  2. Your mobile device to digitally complete any additional insurance forms.
  3. Loose, comfortable clothing. Wear shorts if being treated for your lower back, hip, knee, ankle, or foot (women should wear a sports bra if being treated for a neck, mid-back, or shoulder injury).
  4. Secondary insurance information if you sustained a work injury or were involved in a motor vehicle accident.

Parking at Greenwood – Enter the first level garage just north of the clinic entrance and park in spaces labeled “GPT 24- hour reserved parking”. You will need to walk out of the garage to enter the clinic, as there is no direct access from the garage. There is also free street parking available.

Parking at Ravenna – Enter the commercial garage from 34th Ave NE and park in any of the spaces that are labeled “GPT 24- hour reserved parking”. You will need to walk out of the garage to enter the clinic, as there is no direct access from the garage. There is also free street parking available.

Parking at Lake City – parking is on the South side of the building, there are many spots reserved labeled “GPT 24- hour reserved parking” for patients on the side closest to the clinic. Once you’ve parked, walk around to the North side of the building and someone will buzz you in.
