First Visit Preparation

Welcome to GPT Physical Therapy!

At GPT Physical Therapy, we believe every patient deserves personalized care and a supportive environment. Our dedicated team is committed to helping you achieve your health and wellness goals, using evidence-based techniques and compassionate guidance. From your first visit, you’ll experience a welcoming atmosphere where your comfort and progress are our top priorities. We’re excited to partner with you on this journey.

What to Expect:

  • One-on-one personalized care exclusively with a board-certified Physical Therapist. No aides or care extenders will be used or billed for during your care at GPT.
  • High quality treatment, going beyond the scope of your current issues
  • Approximately 40-45 min sessions (depending on exact needs for each visit)
  • A dedicated administrative and in-house billing team to provide answers to your questions.

Before Your First Visit

You should receive:

  • Two emails from FormDr with your Intake Paperwork & Benefit Letter
  • A welcome phone call 2 days before your first visits from your office
  • A reminder text or email 2 days before your first visit from our EMR Clinicient
  • Potentially a PDF copy of your future scheduled appointments, if requested

How to Prepare for your First Visit

  • Paperwork Please complete our online paperwork with the link sent to you by our front desk team before you arrive for your first appointment. The online registration contains general patient information, medical history, benefit letter and other documents for you to read and sign prior to your appointment. Our online registration links are below if for some reason you did not receive an email from us. You can use your mobile device to digitally complete any additional forms once you get to the office – OR – Hard copies can be supplied if you didn’t get to it ahead of time.
    1. Remember to bring any missing details for your Insurance both Primary and Secondary or claim information if you sustained a work injury or were involved in a motor vehicle accident.
    2. If you did not upload it online, please bring a copy of your insurance cards, ID, any doctor referrals, or post-operative protocols.
  • Location Make sure you know which clinic you are headed to! The address of each of our locations is listed on our home page
  • Parking All our locations have designated patient parking either via a parking lot or garage, check below for more details
  • Payment Be ready to pay any applicable co-pay payment (We accept Visa, Mastercard, Apple Pay, Google Pay, and Personal Checks).
  • ClothingWear loose and comfortable clothing. Wear shorts if being treated for your lower back, hip, knee, ankle, or foot (females should wear a sports bra if being treated for a neck, mid-back, or shoulder injury). Gowns, shorts, and shirts are available if you forget!
  • Download Your Home Exercise Program (HEP) App Ahead of TimeWe have chosen MedBridge as our HEP provider.  Active participation in your home program and implementing recommended lifestyle changes will expedite your treatment and give you long-lasting results. Downloading the app ahead of time so you can quickly start participating in your HEP from day one!
    1. Download the Medbridge GO app from the Apple App Store or Google Play Store to access your prescribed home exercise program after your first visit.

    3. After your Initial Visit, you will receive an email or text with your access code. Click on the access code and your program will be automatically loaded to the MedBridge Go App
    4. We highly recommend using Face ID or a Pin to access your App to speed up entry into the App!

Parking Details

Parking at Greenwood – Park in the “GPT 24-hour reserved parking” spots on the first level of the connected garage just north of the clinic. You’ll need to exit the garage and walk to the clinic entrance; there’s no direct access from inside. Free street parking is also available nearby.

Parking at Ravenna – Use the commercial garage on 34th Ave NE next to the Ridge Pizza and park in “GPT 24-hour reserved parking” spots. Walk out of the garage to reach the clinic entrance. Free street parking is also available.

Parking at Lake City – On 33rd Ave NE, find the parking lot on the south side of the building.  Park in “GPT 24-hour reserved parking” spots closest to the building. Walk around to the north side to enter. Free street parking is also an option.

Parking at Broadview – There are 8 patient-only parking spots just north of the building off 130th Street. Enter through the gate to the left of the building.
